In consideration of being allowed to participate in an on-ice activity at the Tim Hortons Iceplex, I, the undersigned, acknowledge, appreciate, and agree as follows:
If participant is an Adult (18 years or older):
- I am at least eighteen (18) years old.
- I understand the risks involved in participating in this program, including the potential for serious injury, paralysis, or death. I acknowledge that these risks can occur even with proper safety measures in place.
OR
If participant is a Minor (under 18 years old):
- I am the parent/legal guardian of the participant named below (the "Minor").
- I have the legal authority to consent to the Minor's participation in this program on their behalf.
For myself and/or the Minor (as applicable):
Assumption of Risk: I knowingly and freely assume all risks associated with participation in this on-ice activity, even if arising from the negligence of the Organizer or others. I take full responsibility for my/the Minor's participation.
Compliance with Rules: I agree to comply with all stated and customary terms and conditions of Tim Hortons Iceplex. If I/the Minor observes any unusual or significant hazard during participation, I will remove myself/the Minor from the activity and immediately notify the nearest official.
Release and Indemnity: I, for myself and on behalf of my/the Minor's heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Monroe Community Sports Centre Corporation (dba Tim Hortons Iceplex), their officers, officials, agents, employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (collectively, the "Releasees"), with respect to any and all injury, disability, death, or loss or damage to person or property, arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.
